Registration for the 2010 season is underway.  Below you will find a list of the materials needed to register. You can mail the completed forms to the address at the bottom left. We cannot guarantee a spot on a team for this 2010 season.

Pop Warner Age/Weight Matrix

Players must have:

  1. •A copy of your birth certificate or other accepted proof of age (such as a passport).

  2. •A complete Pop Warner Medical Release Form for the player's physical exam (in calendar 2010) signed by a doctor, certifying that the player is physically fit to play tackle football. Do not wait on this one, set up the physical early.

  3. •A complete Player Contract signed by the player and parent/guardian.

  4. •A photograph, head and shoulders, NO HAT, 2"x2" or 2"x3".

  5. •2 copies (on 8 1/2 x 11) of school report cards for all 2009-2010 school year grading periods (thru June).  Must have all quarters or semesters.

  6. •Registration fee.

  7. •All players will be required to attend a conference certification day in late
    August. This is mandatory for all 7,000+ players in the Peninsula Conference.
